Every prescription now evaluated GREEN / AMBER / RED at write-time and dispense-time. RED writes blocked; AMBER auto-escalated to supervising specialist with full context.

ClinicalScopedDb composition pattern
Business Super Admin role can no longer construct a ClinicalScopedDb. Clinical data access is now physically impossible for commercial admins — enforced at the data layer, not just policy.
